Skilled nursing

Our nursing team has both licensed practical nurses (LPN) and registered nurses (RN) that can provide more skilled and complex nursing services to patients when medically required. Skilled nursing often helps people feel better, function more independently and learn to take care of their health needs. Skilled nursing is provided by an RN or LPN in your home or place of residence when prescribed by a physician.

Services may include but are not limited to:

  • Clinical assessment and evaluation
  • Infusion therapies and injections
  • Central line and port maintenance
  • Lab draws
  • Wound care/dressing change
  • Tube feeding
  • Ostomy care
  • Prevention and treatment of skin issues
  • Pain management
  • Other services as needed

Our clinicians can deliver therapies at your home or place of residence and for as long as it is medically necessary and reasonable, including:  

  • Physical therapy
    • Orthopedic Specialty
    • Balance Retraining
    • Mobility Training
    • Pulmonary/Cardiac Rehabilitation
    • Fall Prevention
  • Occupational therapy
    • Arrangement of environment to improve home safety and increase independance
    • Therapeutic exercises related to upper extremity activities and functions

    • Speech Disorder - Dysphasia

Social worker

Medical Social Workers play a vital role in the care team. After a patient is accepted for service by  Select Home Care, a Medical Social Worker may conduct an assessment to help determine patient needs and identify any limitations, barriers, or safety issues that might impede recovery.

They are able to educate patients and families regarding community resources and support, and assist with long range planning, including advance care planning and directives, and overall care opportunities.

Our Medical Social Workers work with the patients and families to keep patients at home in a safe and healthy environment.

Medical Social Services available include:

  • Community resource planning and coordination
  • Advance directives (Power of Attorney for Health Care or Living Will)
  • Assistance in emotional adjustments to life changes
  • Crisis intervention
  • Assistance with applications for financial aid
  • Counseling with life-style changes
  • Teaching regarding coping with loss and change
  • Grief counseling/coping with impending loss
